Pre-announcement: Transitions in Care – Component 2: Best and Wise Practices

The CIHR Transitions in Care (TiC) multi-Institute strategic initiative is co-led by the Institute of Circulatory and Respiratory Health (ICRH), Institute of Health Services and Policy Research (IHSPR), Institute of Human Development, Child and Youth Health (IHDCYH), and the Institute of Indigenous Peoples’ Health (IIPH), in collaboration with the Institute of Aging (IA), Institute of Cancer Research (ICR), Institute of Gender and Health (IGH) and Institute of Musculoskeletal Health and Arthritis (IMHA).

Background

  • The Best and Wise Practices Grant is Component 2 under the CIHR TiC multi-Institute initiative.
  • The focus of the Best and Wise Practices Grants is to support groups of researchers and knowledge users to identify best and wise practices in two (2) or more of the identified TiC focus areas. The grants will also provide support to groups that require additional evidence gathering and/or the building of relevant relationships and partnerships to address challenges, gaps and/or barriers related to improving care transitions and mitigating negative patient outcomes resulting from poor transitions. Overall, this competition will support planning and information-sharing activities, partnership development as well as evidence gathering to facilitate the collection of data and building of relevant research capacity in preparation for the launch of future TiC funding opportunities.
  • Recognizing that equity is an integral part of a sustainable health research ecosystem, these grants are committed to support Indigenous Health Research and specific sex and gender minorities.   

Funds available

The total anticipated amount available for the RFA is $1,200,000, enough to fund up to twelve (12) grants.

The specific objectives of The Best and Wise Practices Grants are to:

  • Develop relevant research capacity leading up to TiC Component 4: Team Grants Phase 2;
  • Inform actionable health system changes within the TiC Focus Areas by supporting systematic identification and synthesis of transitions in care knowledge gaps and/or identification of effective evidence-based interventions and/or policies;
  • Facilitate the identification of relevant transitions in care stakeholders and partnerships, with a particular focus on partnerships that can support community-based research and approaches that have potential for equitable spread and scale between communities, at a regional and/or national level.

Research Areas

This RFA will support applications centered on the intersection of two (2) or more of the TiC Focus Areas. In addition, funding is available to support projects that are relevant to the following specific research areas:

  • Indigenous Health Research
    Indigenous Health Research as defined as any field or discipline related to health and/or wellness that is conducted by, grounded in or engaged with, First Nations, Inuit or Métis communities, societies or individuals and their wisdom, cultures, experiences or knowledge systems, as expressed in their dynamic forms, past and present.
  • Individuals who identify as transgender and individuals living with an intersex condition
    Individuals who identify as transgender and individuals living with an intersex condition can face additional challenges to successful care transitions. Note that, in the case of individuals who identify as transgender, this research area is not restricted to care during gender transitioning, and can consider transitions in care at any stage of life.
