Funding Opportunity Live: Transitions in Care Evaluation Grants

CIHR, in partnership with the Rossy Family Foundation, is pleased to announce an upcoming Evaluation Grant funding opportunity under the Transitions in Care (TiC) Initiative. TiC is a multi-Institute strategic initiative co-led by the Institute of Circulatory and Respiratory Health (ICRH), Institute of Health Services and Policy Research (IHSPR), Institute of Human Development, Child and Youth Health (IHDCYH), and the Institute of Indigenous Peoples’ Health (IIPH), in collaboration with the Institute of Aging (IA), Institute of Cancer Research (ICR), Institute of Gender and Health (IGH), Institute of Musculoskeletal Health and Arthritis (IMHA) and the Institute of Neurosciences, Mental Health and Addiction (INMHA).

This competition will support the rigorous evaluation of implemented activities such as interventions, tools, practices, policies, technologies, programs and/or other solutions that address transitions in care challenges. The focus of the Evaluation Grants is to support researchers and knowledge users to formally evaluate, validate and/or demonstrate the effectiveness, appropriateness and/or impact of implemented transitions in care activities to create high-quality, real-world evidence, relevant to transitions in care, overall health and/or health system improvement.

The Evaluation Grants are expected to:

  • Develop relevant research capacity leading up to future TiC funding opportunities;
  • Inform the implementation or adoption of transitions in care activities within the health system to optimize outcomes of patients experiencing transitions in care by generating high-quality, real-world evidence;
  • Identify clear and actionable health system changes, relevant to transitions in care.

Research Areas

This funding opportunity will support applications centered on the intersection of two or more TiC Focus Areas. In addition, funding is available to support projects that are relevant to the following specific research area:

  • On-Campus Post-Secondary Mental Health Services: Identifying post-secondary students at risk of mental health problems and transitioning students-in-need into and through on-campus mental health services and/or to community-based care creates challenges for students, families, campuses and communities that may lead to serious but avoidable gaps in care and poor mental health outcomes for students.

Funds Available

The total amount available for this funding opportunity is $3,200,000, enough to fund approximately 16 grants. The maximum amount per grant is $100,000 per year for up to two (2) years for a total of $200,000 per grant.

Of this $3,200,000:

  • $400,000 is available to fund approximately two (2) grants specifically relevant to On-Campus Post-Secondary Mental Health Services

  • Program Launch: Fall 2018
  • Application Deadline : Winter 2019
  • Notice of Decision: Spring 2019
  • Funding Start Date : Spring 2019
